Our current 9 question general interest survey is on the relationship of our childhood training to our adult behavior.  One additional 10th question asks if you would be interested in receiving the results by email.  If you choose to not provide your email address, your responses will be entirely anonymous.  This current survey will be open until March 31, 2020.

The survey should take about two minutes to complete.

The survey is in two web locations.  The first is through our in-house survey tool.  This tool is free of advertising but is in a format that requires your web browser to have the ability to run Adobe® Flash Player.  The second is through SurveyMonkey®; a third party, software as a service company, with abundantly punctuated advertising.
